That second one reminds me of an incident early in the (current) war, when a photographer shot a series showing American GIs watching over a large group of Iraqis -- the two guys standing, the Iraqis sitting on the ground. Apparently, in one shot, one of the Yanks was looking off in the wrong direction, so the photog cloned him in from another shot where he was facing the crowd. Unfortunately for him, some of the folks in the crowd had changed positions and so showed up twice in the shot. It cost him his job.
